For and in consideration of my being allowed to participate in any way in any FAYETTEVILLE 
DISC ASSOCIATION ACTIVITIES, I agree to the following:

I understand dangers may be caused by my own actions, or inactions, the actions or inactions 
of others participating in the Activity, and the condition of the premises. I understand the nature 
of Ultimate and of FAYETTEVILLE DISC ASSOCIATION Activities and acknowledge my 
experience and capabilities and believe I am qu¥ified to participate in such Activity. I further 
acknowledge that I am aware that the activity will be "conducted in facilities open to the public 
during the Activity. I further agree and warrant that if, at any time, I believe conditions to be unsafe, 
I will immediately discontinue further participation in the activity.

I FULLY UNDERSTAND that: (a) Ultimate Frisbee and FAYETTEVILLE DISC ASSOCIATION 
ACTIVITIES involve risks and dangers of SERIOUS BODILY INJURY, INCLUDING PERMANENT 
DISABILITY, BLINDNESS, P ARAL YSIS AND DEATH (hereinafter referred to as "risks"); 
(b) the aforementioned Risks may occur due to the nature of the Activity or the negligence of the 
"Releasees" named below; (c) there may be other risks and social and economic losses either 
not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME 
ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES 
incurred as a result of my participation in the Activity or condition of the facility.

And in addition to the above, and for the same consideration previously stated, I HEREBY 
AGREE TO INDEMNIFY, RELEASE,  DISCHARGE, COVENANT NOT TO SUE, AND AGREE 
TO HOLD HARMLESS, the FAYETTEVILLE DISC ASSOCIATION AND THE OWNER(S) OF 
THE FACILITIES ON WHICH THE ACTIVITY TAKES PLACE, any of their respective administrators, 
directors, agents, officers, volunteers, and employees, other participants, any sponsors, and 
advertisers (each considered one of the "Releasees" herein). I hereby agree to indemnify, defend, 
protect, and hold harmless the Releasees for or against any and all manner of claims, causes of 
action, liability, or damages on account caused or alleged to be caused in whole or in party by the 
negligence of the "Releasees" or otherwise, including negligent rescue operations, and arising out 
of any accident, injury or damage to me occurring while I am on or about the property or participating 
the FAYETTEVILLE DISC ASSOCIATION activity. 
I further agree that if, despite this release, I, or anyone on my behalf makes a claim against any of the 
Releasees named above, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS EACH OF THE 
RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS LIABILITY, DAMAGE 
OR COST ANY MAY INCUR AS THE RESULT OF ANY SUCH CLAIM.

This release shall remain in full force and effect until or unless revoked in writing by both myself and the 
Releasees; and in any event, it shall remain in full force and effect so long as I shall or may go upon the 
property or facilities used in FAYETTEVILLE DISC ASSOCIATION Activities.

IN WITNESS WHEREOF, I hereunto set my hand, and have agreed to the terms of this instrument after 
having carefully read it in full, this day of, 2002.
